How to Get Ready

You will need to do the following when you come in for your first visit:

• Bring all related medical records and original diagnostic films.
• Check your insurance plan by calling our office.
• Bring a valid insurance card and drivers license.
• Bring a list of your current medications.
• Bring the referral from your primary care physician, if required.
